Like when you write a pairing with the slash mark. As in Kate/Jack or Kara/Lee or... Spock/Kirk. Hahaha.
I think it started out meaning any romantic relationship that wasn't canon. But, I guess it kind of evolved to mean strictly homosexual relationships, and now that there are characters in canon, like Willow and Tara from Buffy or Brian and Justin from Queer as Folk, the term applies to those same-sex relationships as well, out of sheer convenience.
